Formula Fitness provides personal training, small group fitness training, and related wellness services at our Los Alamitos, California location. All services are subject to availability and scheduling.

You confirm that you are in good physical health and have consulted with a physician before beginning any exercise program. You understand that physical activity carries inherent risks, and you voluntarily assume all risks associated with participating in our programs. 4. Waiver of Liability

You release Formula Fitness, its owners, employees, and trainers from any claims arising from your participation in our services, except in cases of gross negligence. This includes injuries, accidents, or health issues that may occur during training.

5. Cancellation Policy

We require 24 hours notice for cancellations or rescheduling. Late cancellations or no-shows may result in forfeiture of the session. Purchased sessions are non-refundable and non-transferable unless otherwise specified.

6. Payment

Payment is due at time of purchase for all packages and services. Memberships are billed according to their specified billing cycle. You agree to provide accurate payment information and authorize recurring charges if applicable.

7. Photography & Media

By participating in our programs, you consent to photography and video recording for marketing purposes. If you do not wish to be photographed, please inform us in writing.

8. Age Requirements

You must be at least 18 years old to use our services. Minors may participate only with parental consent and supervision as required.
